IMT Corporation of Ingersoll, Ontario, was recently fined $60,000 (Canadian) for a violation of the Occupational Health and Safety Act (OHSA) that caused an injury to a worker. The violation occurred in March 2009 at the company’s Port Colborne drop forge facility, which supplies forgings for captive and general industrial use.
The violation occurred when a worker was greasing a machine and accidentally stepped on the activation pedal. The worker’s forearm was crushed when the ram descended. A subsequent investigation by Canada’s Ministry of Labor found that the machine had not been locked out prior to servicing. IMT pleaded guilty to failing to ensure the machine was locked out before being greased.
